We are now requesting that you use our email system to request blue and yellow school forms (Florida Forms DH 680 and 3040). We ask that you email the 'Front Desk' email address with the patient's name and date of birth and the requested forms needed. Please do not email the front desk email address with questions about medical problems or prescription refills.
Dr. Bradford does not check his email address on the weekend or at night on weekdays, so if you have a medical problem that you would like to speak to him about, please call him at those times. Also, be aware that his email address does not get delivered to his phone or another electronic device, so he may not receive your email until after the morning/afternoon schedule of patients.
